Perhaps your physician has asked you for a copy of your X-ray or other kind of radiological film. If you've had multiple exams, make sure you know which exams you need. Then use one of the two methods below.
- Call us - The Radiology film library for Willamette Valley Medical Center can be reached at 503.435.6420. After you call, we'll have a copy of your latest radiology test available for you to pick up. Normal front desk business hours are between 7:30 am and 5:00 pm. When you come to pick up your X-ray, we'll ask you to complete medical records release form. This form is necessary for your security as well as our records.
- Fax us- Our fax number in the Radiology Film Library is 503.435.6384.
If you (or your doctor) request your film via fax, we'll need your complete name, date of birth, and exams needed. With 24 hours notice, we'll pull the master set of films and have them available at the main counter of the Radiology Outpatient Desk. When you come to pick up your X-ray, we'll ask you to complete medical records release form. This form is necessary for your security as well as our records. If you can't come to the hospital in person, you can obtain one of the forms from us by calling 503.435.6420, and we'll be happy to fax or mail the form to you. Then, you should fax or mail the completed and signed form to us.
