Maintaining patient privacy and adhering to the laws regarding patient privacy is important to us.
In order to release medical records, we require the completion of an HIPAA compliant release of information form that must be mailed, faxed or delivered to the hospital (no emails accepted). The release form can be downloaded below, along with the instructions on how to complete the form. Our most frequently asked questions are also available online.
