Monday – Friday, 8 a.m. – 4 p.m.
In order to release medical records, we require the completion of an HIPAA compliant release of information form that must be mailed, faxed or delivered to the hospital (no emails accepted).
The release form can be downloaded below, along with the instructions on how to complete the form. Our most frequently asked questions are also available. If you have a question that we do not address, please call our office during business hours. We would be happy to answer any additional questions.
MEET STACI COY, DIRECTOR OF HEALTH INFORMATION MANAGEMENT AND HIPAA PRIVACY OFFICER:
Grew up in Memphis, and loves the Oregon weather.
LOVES doing family history research.
Loves to travel and spent two years in Southeast Asia working in refugee camps.
What do you love about working at Willamette Valley Medical Center?
I love that when we meet to make decisions, we always ask, “What’s the best practice?” — not just “What is good enough?” We want to be the best at everything.
As our privacy officer I want to make sure people feel that their health information is well protected.
I love my job, and feel very blessed to have a job I adore. My team makes all the difference. My entire HIM department was nominated as “employee” of the year because they work so well together as if they were one. I am blessed to associate with them every day.
